Job Summary: We are seeking a compassionate Licensed Practical Nurse (LPN) to provide skilled nursing care to patients in the home setting. As a skilled nurse caregiver, you will work closely with patients and families to provide high-quality care and support. Responsibilities:

• Execute physician-prescribed plans of care and document care provided in a system of record.
• Administer prescribed medications, treatments, and therapies.
• Conduct patient assessments and coordinate care.
• Promote health education and training to family members.Requirements:
  • Good standing license (LPN) in the state of practice.
  • Current CPR certification.
