Job Description:Job Description:Colorado Physician Partners is looking for a Nurse Practitioner to join our growing Harvard Avenue Internal Medicine Practice. Our group of providers includes 2 Physicians and 2 APPs and is consistently a top qualify provider for primary care. Colorado Physician Partners (CP2) is a member of the independent practice association (IPA), PHPprime. PHPprime provides wrap-around services to help make our practice successful, including care coordination, practice transformation coaching, risk adjustment coding education, population health management, data and analytics, specialist network management and large group contracting.Location850 East Harvard Avenue, Denver, CO 80210The Position:

• 1.0 FTE opportunity
• Up 14 patients per day
• Seeing Ages 20+
• Will work closely with partner physicians to manage a panel of primary care patients with a strong emphasis on prevention of illness.Qualified Candidates:
  • Master of Science degree in Nursing from an accredited program
  • ANCC or AANP Certification
  • Provider must meet employer credentialing standards.Salary Range & Benefits
    • $100,000.00 to $135,000.00 annual base pay 4 day work week
