The Adams County Health Department Nursing Division is comprised of Nurse Family Partnership, Nurse Support Program, Harm Reduction, Sexual Health, Immunizations, Child Maltreatment and Fatality Prevention, and Care Coordinator for Children and Youth with Special Health Care Needs. The Nursing Division is made up of clinical support staff, medical assistants, harm reduction specialists, operations support, nurses, advanced practice nurses, and midwives.

The anticipated hiring range for this role is $79,274.02 - $91,165.123 annually. The full salary range is $79,274.02 - $118,911.04 annually.
Examples of Duties for Success

The Sexual Health (Clinical) Program Manager provides program oversight, clinical leadership and supervision for public health professionals and program specialists within the Sexual Health & Harm Reduction programs, directing integration of program services in brick-and-mortar clinics, mobile units and field delivery services. The position is responsible for budget monitoring, policy and procedure standards, program reporting to funders and evaluates performance of direct reports and program operations. Recommends and administers actions impacting employee pay and status, hiring and discipline. Serves as a resource or expert in program areas providing technical guidance and education to assure program success.

The Clinical Program Manager works collaboratively with the Harm Reduction (Mobile) Manager to provide Sexual Health & Harm Reduction Services in the clinics, mobile units and field delivery services. The Clinical Manager and Mobile Manager ensure the clinic and mobile services are complimentary and meet all requirements for services delivery including clinical and education standards and grant deliverables. The Clinic and Mobile Managers work collaboratively with the Sexual Health & Harm Reduction Clinical Advisor on the development of evidenced-base policies and procedures.

The Clinical Program Manager maintains fidelity to the programming model, standards of practice, and implementation of evidence-informed or evidence-based population health. The manager uses principles of supervision, reflective practice, and staff development to provide primary support and appropriate oversight to staff.

Leadership

  • Manages day-to-day work related to the provision of care in brick-and-mortar clinics, mobile units and field delivery services.
  • Uses data analysis to develop community-specific program services related to contraception, STI testing, treatment and education, substance use education and overdose prevention. Incorporates removal of barriers to accessing program services.
  • Seeks out program funding, manages grant budgets and deliverables to maintain program services.
  • Effectively provides direct supervision for program staff, including nurses, program support specialists and students which includes coordination of staff training, assigning and reviewing work, and evaluating performance independently or in conjunction with higher level supervisors/managers.
  • Develops and oversees robust staff development, including required orientation and training opportunities for maximum program delivery.
  • Acts as a resource to the program, Nursing Division, entire health department, and county offices for support in nursing services, education, and training. Provides content expertise and recommendations to nursing leadership, including within the Health Department leadership structure, county partners, and Board of Health.
  • Provides continual idea generation to assess existing and emerging health risks to Adams County and the region and work with the Population Health Epidemiology data team to investigate and quantify those risks and Health Policy and Health Equity to make system changes.
  • Coordinates with supervisor to respond to the changing public health environment through problem-solving and adapting tactics as needed using change management tools and best practices
  • Regularly reports the status of related efforts and impacts. Community Engagement:
    • Coordinates and integrates program activities with other nursing programs, ACHD divisions, allied agencies, service providers, and community groups.
    • Develops partnerships with community organizers, advocacy groups, cross-sector partners and other local health departments to elevate solutions for addressing population health.
    • Builds and maintains relationships with community, government, healthcare partners, business leaders and partner organizations to understand community assets and priorities, create connections across organizations, and ensure achievement of shared objectives.
    • Supports diverse perspectives in developing, implementing, and evaluating policies and programs that affect population health. General and Administrative:
      • Works with process improvement staff and division director to develop and implement strategies for continuous process improvement and development/maintenance of outcomes-based performance tracking, and evaluation activities for all aspects of program work.
      • Investigates way to collect, evaluate, and analyze qualitative and quantitative data within program workflows to inform equity related decision-making processes and maximize resources with the department and community.
      • Incorporates data, community input, and evidence-based guidelines to make recommendations and communicate strategies to improve outcomes, considering the varied needs of specific populations, such as issues related to age, gender, culture, and health inequities.
      • May lead processes to identify and secure funding to support program work including addressing health equity, data collection, and systems for improved services.
      • Responsible for monitoring compliance and reporting deliverables with program contracts, budgets, expenditures, and billing processes of program.
      • Provide routine surveillance of potential risk. Identifies risks/solutions to reduce inequities.
      • Perform other related duties and responsibilities as required Additional Duties:
        • Assists in the orientation and training of new employees, students, and volunteers.
        • Participates in quality improvement projects.
        • Maintains electronic health record (EHR) proficiency and skills as necessary for daily duties. Supervisory Responsibilities:

          Exercises direct supervision over supervisory staff, licensed staff, and leads. Effectively provides direct supervision for program staff, including nurses, program support specialists and students which includes coordination of staff training, assigning and reviewing work, and evaluating performance independently or in conjunction with higher level supervisors/managers.
